SAP SELECTION IN FMCSA CLEARINGHOUSE & NEXT STEPS

Please follow these steps to designate us as your Substance Abuse Professional (SAP) within the FMCSA Clearinghouse system:

1. Access the Portal: Go to the official FMCSA Drug and Alcohol Clearinghouse: https://clearinghouse.fmcsa.dot.gov/

2. Locate Violation & SAP Selection: Find your violation notice within the Clearinghouse. Scroll down to the designated area for selecting a SAP.

3. Search for SAP: In the SAP selection section, enter the following information exactly:

• SAP Name: Uffens, Rachael

• Address: 1921 S Ama School Rd, Suite 312. Mesa, Arizona 85210.

4. Initiate Request: Select my profile and submit the request.

Upon your request submission, I will receive notification and promptly send DocuSign forms on my end.

Next Steps: To initiate your case file and prepare the necessary paperwork, we require two items from you:

• A copy of your Commercial Driver’s License (CDL).

• An email from you confirming your details.
